Moldavite is a form of tektite, which is formed when a meteorite strikes the earth's surface, it generates a huge amount of heat & pressure, which will vaporise & melt the surrounding rocks and soil. This results in super-heated debris flying into the atmosphere, while it is in the atmosphere is cools and solidifies into this glass like form as it falls back to Earth.

 

The Meteorite itself hit what is now Nordlingen, Germany, around 14.8 million years ago, leaving behind a huge crater known as the Ries Crater, this crater is around 24km wide! The moldavite landed in what is now Czech Republic, and cannot be found anywhere else in the world, which adds to its rareness.
